Chapter 37.5. Health Care Service Prior Authorization

Sec. 6. As used in this chapter, "participating provider" refers to the following: (1) A health care provider that has entered into an agreement with an insurer under IC 27-8-11-3. (2) A participating provider (as defined in IC 27-13-1-24).
